Submit a Concern Referral

If you know a student who may be experiencing signs of distress or are a student experiencing distress yourself, please submit a student of concern referral form.

Note: If you are referring yourself, please list your name in the “person referred” portion of the form.

If you would like to submit a referral form for someone who works at Tennessee Tech, please submit an employee of concern form.

While anonymous reports are accepted, you are STRONGLY encouraged to provide your name and contact information so the appropriate department or team can request additional information or follow up.

What to Expect After You Submit a Concern Referral

Once you complete a referral form, the “person referred” will receive an initial email by the following business day. Phone calls and texts are sent as well if the person does not respond to the initial email. If requested, a CARE Team representative will notify the referring party once initial outreach to the student has been completed. However, due to privacy laws, the CARE Team is limited in our ability to share information with the referring party after the initial outreach.

The first CARE Team member a student is likely to meet with is the Clinical Coordinator. Students are encouraged to share their experiences and concerns with the Clinical Coordinator. The next steps will depend on each student’s unique circumstances but will include connection to appropriate resources and follow-up.
