Submit a Person of Concern Report

If the person does not require immediate assistance or consultation with the Center for Counseling and Mental Health Wellness, submit a concern referral.

Submitting a concern referral allows trained professionals to evaluate the concern and follow up with the individual to provide resources and support.

For students, referrals will be reviewed by the CARE Team. Human Resources will review employee referrals.

While anonymous reports are accepted, you are strongly encouraged to provide your name and contact information so additional information may be requested and follow-up is possible.

About the CARE Team

The CARE (Coordination, Assessment, Response, and Education) Team evaluates and responds to students experiencing distress by connecting them with resources to support their well-being. Students referred to the CARE Team will be contacted by the appropriate Team member(s), and progress updates are shared regularly among Team members until the student’s circumstances no longer necessitate CARE Team intervention. Though the Team meets weekly, information is shared and follow-up among members and with students is ongoing throughout each week.

The team consists of:

  • AVP/Dean of Students (chairperson)
  • Center for Counseling and Mental Health Wellness Director
  • Clinical Coordinator
  • Chief of University Police
  • Title IX Coordinator
  • Associate Provost
  • Accessible Education Center Director
  • Assistant Director of Residential Life
